Why enclosed spaces kill
Most enclosed-space deaths on ships happen when someone enters an untested tank, or when a second person goes in to rescue the first without breathing apparatus. Oxygen can be low, cargo vapour can be toxic, and a space that was safe an hour ago may not be safe now. IMO and company SMS require a permit, tested atmosphere, a standby person and a rescue plan before anyone goes in.
How to use this page
This is a memory checklist only. It does not replace the ship’s official Enclosed Space Entry Permit. If any box cannot be ticked, do not enter. Atmosphere limits and test methods must follow the company SMS and the equipment maker’s instructions.
Who should use it
Useful for officers briefing a tank-entry job. The official permit in the SMS remains the only document that authorises entry.
